Our Mission
Greenacre Elder Home Health Foundation is dedicated to achieving rights of older persons and improving quality of life of older persons at home and abroad. Our goal is to promote, maintain and restore older person health in home setting. We desire to maximize the skills and experience of our team to care for older persons’ and promote their independence to age-in-place while providing them an alternative to nursing home placement.

At Greenacre Elder Home Health Foundation we pride ourselves in our ability to screen and hire quality skilled and non skilled caregivers. We verify all licenses and conduct a FDLE background screening on all applicants prior to hiring. We maintain high ethical work, standards and strictly adhere to all state and federal regulations governing our industry.

US Healthy Aging projects

Our healthcare professionals continues to make healthcare accessible to older persons by providing needed home based care and education to older persons in the US, encouraging aging-in-place as an alternative to nursing home placement.

Services we provide include Registered Nurse assessment and Observation, Comprehensive Diabetic Management, Pulmonary management, Vascular/ Wound management, Cardiac/Stroke Recovery, Post-Operative care, Physical and Occupational Therapy and Home Health Aid.

To make healthcare affordable, we utilize knowledge of diverse health insurance programs as available and suitable and tailors it to our individual client need.

US Healthy Aging projects

As part of our main objective extend our services to older persons abroad- especially in Africa, Greenacre Elder home health Foundation partners with Indigenous organizations like Mission to Elderlies Foundation www.missiontoelderliesfoundation.org to implement health aging project in Nigeria and Africa region.

The healthy aging project provides community-based, hospital-based and home-based services to older persons who are the beneficiaries. These projects are done across communities in Isseke, Ukpo, Afikpo, Uburu, Ezzamgbo, Mbaitolu Communities among others in Nigeria

Partnering with Health Insurance Program, we enroll older persons into health insurance program to reduce out of pocket expenditures on healthcare by 90%, where with 15USD, an older persons is enrolled into insurance program to access healthcare at tertiary level annually.

On community based healthy aging projects, support groups of older persons and caregivers forum are set up in communities through which healthy aging information are provided and support provided for games and recreational activities, an also for cultural and inter-generational activities. This encourages healthy aging, social inclusion, prevention of ageism, isolation, anxiety and depression among older persons.

On home-based care, caregivers and community volunteers are trained and supported with stipends to visit homes of older persons’ and beneficiaries to provide assessment, education and linkage to hospital on need. In addition, gender based violence or violence against sexual orientation are assessed and needed legal support provided.

On hospital-based care, we support set up of healthy aging clinic in existing community hospital to serve older persons on health care assessment, medication and walking aid provision, eye care and active referral.

In addition, we support vulnerable households of older persons with Household economic strengthening (HES) program that supports older persons and their caregivers with Income Generating Activities (IGA).
